We would like to seek and clarify your views in relation to the Primary Care Medical Services you currently receive from your GP practice.
Six GP practices currently operated by ACHSCP are part of a procurement process which may change the management structure. This might result in changes to how some of the services from your practice are delivered to you.
To ensure we are as informed as possible about the views of the patients from each practice, ACHSCP have summarised National and local feedback from recent consultations into a document, ‘A summary of patient consultation relating to Primary Care Medical Services’. We have attached an infograph of the information in that document and would like to ask you; does this summary continue to represent your views and is there any additional information you would like to add? We would really appreciate your feedback which will be used to help shape services going forward.
