The Partnership’s Annual Report 2018-19 has been commended by the Integration Joint Board.
The report outlines the extent to which the organisation has met the aims and objectives laid out in its Strategic Plan covering that period, along with an overview of spending.
The document looks at performance against a range of national health and care indicators – and details a range of activities undertaken during the year, under the heading “That was the year that was”.
The report also looks at the many areas of good practice which are under way and looks to the future in terms of areas for improvement.
Highlights for the year include:
- continued improvements in delayed discharges
- reduction in admissions from A&E to a hospital bed
- implementation of a range of key strategies
- progress on a number of transformation projects
- approval of a Workforce Plan.
Areas for improvement include reducing the falls rate per 1,000 population aged 65+, increasing the percentage of adults with intensive care needs receiving care at home, and reducing the number of A&E attendances.
IJB chair Cllr Sarah Duncan commended the accessibility and clarity of the Annual Report, adding: “We have improved on nearly every indicator, which a testament to the hard work of the Partnership staff, and we are following up on those performance indicators which are causing concern.”
