Community Link Practitioners are part of our ACHSCP transformation programme. This resource is designed to help address health inequalities by building capacity in General Practice. The programme aims to reduce pressures on mainstream primary and community care services by embedding Link Practitioners in the community, local GP practices and our locality teams.
As part of the programme, person-centred wellbeing support is being organised through a dedicated member of staff in each GP practice called a Link Practitioner. They aim to improve people’s resilience by supporting them to link more closely with the opportunities and resources within their own communities.
