Our Communities

 

About Communities 

The Communities team consists of the Public Health and Wellbeing teams. We work in partnership in the delivery of the strategic priorities of the Aberdeen City Health and Social Care Partnership with a focus on prevention and early intervention, improving population health and wellbeing, reducing social isolation and health inequalities

The team also leads on the development, implementation and evaluation of key public health priorities, Locality Plans, and coordination of local response to national consultations, and delivery of our Stay Well Stay Connected programme.

 

 

Our Contribution 

The communities team contribute to both local, regional and national policy and action plans such as:

 

Delivery Plan / Key Priorities 

  • Addressing health inequalities and tackling poverty
  • Addressing the obesity epidemic and promotion of active lives (physical activity)
  • Reducing smoking prevalence across the population and preventing e-cigarette and emerging tobacco produce use vaping uptake by children and young people
  • Improving health and wellbeing for children and young people
  • Work with NHSG Public Health Directorate and alongside other Grampian Health and Social Care Partnerships to explore the development of a public mental health approach for Aberdeen City
  • Delivery of Stay Well Stay Connected programme of holistic community health interventions focusing on social isolation.  
  • Community and professional capacity building
  • Lead on increasing and diversifying the membership of our Locality Empowerment Groups and increasing wider community participation in locality planning
  • Co-design Aberdeen as an Age Friendly City which helps people to age well and make full use of their retirement and later years in the best possible health and wellbeing. We also want to encourage the development of a social movement to enable people to stay well and stay connected within their communities
